Childbirth is a much less messy and painful business for Sims, which is appropriate for a video game.
Twins are common, but only under certain circumstances. Individual Sims can have one of six "Aspirations", and one of the choices is "Family". Aspiration points can be collected and redeemed for awards, and one of these is for twins, making it almost a certainty, especially if both mother and father have the award. This wasn't the case for Pinkey and Pindle, but they do both have "Family" aspiration.
Visiting sims, like Zaria, will autonomously interact with the babies, picking them up off the floor, cuddling them and taking them to the fridge for a bottle. They usually (but not always!) put them back in a crib afterwards. Zaria cuddled Pinkney for a while, then put him in a crib before wandering off towards home, which was handy while I had Pinkey caring for Piper.
I've got babies down to an exact science in the game. As soon as they're born, the mother does six interactions of cuddle or play, gives them a bottle, and puts them in the crib. After about six hours either the infant will get a dirty diaper and their thumbnail will turn orange, or a parent will autonomously decide to get them out of the crib. In either case I do another six cuddle/play interactions, give them a bottle, and change their diaper (if they didn't need it when picked up, they invariably do at this point). The interactions mean that the infant has a good friendship level with their parent(s) by the time they become a toddler, which is necessary to avoid a visit from the social worker. It takes three Sim days for babies to become toddlers, and you get 24 hours notice at 6pm the day before. Babies always grow to toddlers at 6pm, regardless of when in the day they were born.
